- A statement, made under penalty of perjury, that the above information is accurate and that you are the copyright owner or authorised to act on the copyright owner's behalf.
- Your physical or electronic signature.

How the directory handles takedown requests
Every DMCA notice is logged against the requester contact information, the claimed copyright work and the alleged infringing material. The directory acknowledges receipt within 5 working days and reviews the claim within 10 working days. When the claim is valid, the affected record is removed from the staged snapshot and the next catalogue build will not include it.
Counter-notice procedure
If a record owner believes the takedown was issued in error, they can submit a counter-notice that includes their contact information, a statement under penalty of perjury that the material was removed by mistake, and their consent to the jurisdiction of a federal court. The directory forwards the counter-notice to the original claimant and restores the record after the statutory waiting period unless a court action is filed.
